Advance Directives for Mental Health Treatment

An advance directive is a legal document that tells what healthcare services you want to receive if an illness makes you unable to make decisions for yourself. 

Each state has different rules about how advance directives must be written and approved. To make sure you have all the information you need, check with your state's protection and advocacy program, a lawyer, paralegal, or advocate. Please note that only attorneys, however, can offer legal advice.
