Keeping track of your sleep is an important part of maintaining good sleep habits.  This sleep diary developed by the National Sleep Foundation can help you do that.  It only takes a few minutes each day to complete.  It is best to keep it in a convenient place to help you remember to complete it. 

  • Complete the sleep diary for seven days, or copy it for longer periods of time
  • Examine the sleep diary to see if there are any patterns or practices that may be contributing to your sleep problems
  • Discuss problems with your health care provider and share your diary with him or her.

This sleep screening form can be used to help identify daytime sleepiness, which may be an indication of a sleep disorder.  This scale is not meant to diagnose or treat any illness or replace the consultation of a health professional.
